Replace lost or stolen licences 

Replacing a lost licence

A duplicate copy of your driver's licence may be obtained by attending a Licensing centre. You will need to produce proof of identification. (Originals only)

Application may also be made by post. You will need to provide your:

  • Driver’s licence number,

  • Full name,

  • Date of birth,

  • Address and;

  • Enclose supporting documentation such as marriage certificate, birth certificate or passport.
    Copies of these documents must be certified as true and correct copies by a Police Officer or Justice of the Peace.

The cost of a duplicate licence is SSI - Duplicate driver licence $30.40 , for Seniors and Pensioners it is SSI - Duplicate driver licence (Seniors and Pensioners) $15.20 . Aged Pensioners do not have to pay this fee.

Cheques or money orders should be made payable to 'Department of Transport - Licensing Services'.

If we do not have a photo and signature on our records then you will be required to attend a Licensing centre or complete a Photokit (interstate or international applications only). Please contact us beforehand if you are unsure if we have your photo and signature on record or not. You will need to provide your full name, date of birth and driver's licence number when making this enquiry.

Replacing a stolen licence

If your driver's licence has been stolen, you can receive a replacement licence free of charge by supplying a police crime report number along with your application.

If we do not have a photo and signature on our records then you will be required to attend a Licensing centre or complete a Photokit (interstate or international applications only). Please email or call us beforehand if you are unsure if we have your photo and signature on record or not. You will need to provide your full name, date of birth and driver's licence number when making this enquiry. 

You can obtain a replacement for your stolen licence by attending a Licensing centre or in writing. You will need to supply the following information:

  • Full name.

  • Date of birth.

  • Licence number (if known).

  • WA residential address.

  • Current postal address (overseas/interstate).

  • Police report or police report number.

You can submit this information using an Application for Duplicate / Certified Copy of Motor Driver's Licence form.
